INSTRUCTIONS

Include a copy of your IRS determination letter as a 501 (c) (3) qualifying organization.

Include copies of your current budget and your most recent financial statements (balance sheet, report of income and expenditures) for your most recent fiscal year and such interim statements as you may have available. 

COMPLETE THE APPLICATION

In the upper right-hand corner, please indicate whether this is your first grant application and list  grants previously received from the Foundation.

Name  Organization name as is appears with the IRS.

Project Title  In less than 5 words, name the project (e.g. computer purchase).

In 20 words or less summarize the project. The directors will refer to these words several times during the review process.

Describe how the grant funds will be utilized to enhance the Rock Island Community. This provides an expansion of the summary of the "20 words or less" question.

 

List the sources of your funding and give your fiscal year

List the names of persons on your board of directors/trustees

Include information that you want each board  member to know including a brief mission statement for your group

Signatures reflect approval by the leaders of your organization

Provide name, address, phone and email for the contact person.

Briefly, Doris & Victor Day Foundation, Inc.

  • funds organizations possessing a 501 (c) (3) letter from the Internal Revenue Service

  • funds programs that meet basic human need and help to create a better Illinois/Iowa Quad Cities region

  • considers applications from previous grantees only if written reports on prior grants have been received

 

DECISION PROCEDURE

The Board of Directors sees every grant application.  The single-page, application/cover page is reproduced and is read by each director prior to the first session of grant decision meetings.

At that first meeting, the directors select those applications that will be given further study.  In some cases, applicants will be asked to provide more information about the organization and the project.

All grants are summarized on a meeting work sheet that includes organization name, grant title, the 20 word summary, and the amount requested.

Supplementary materials (newsletters and brochures) may be provided with the application. Those materials are placed in the organization's file to be accessed during board meetings.

NOTE: You may create your own application with your word processor and adjust the spacing to accommodate the information you provide. However, please present your responses in the order found on the application contained in this report and respect the ONE-PAGE limitation.

 

(Several granting organizations have agreed in the creation of the single-page application to be utilized as the cover page in the application process.  Materials prepared for other grantors are accepted by the Foundation.)
